Submit a New Record for Registration

The Arkansas Department of Health receives and registers new birth, death, marriage, and divorce records from:

Paper Submission

Authorized representatives of any of the above may obtain copies of all requisite forms, including blank certificates, from their local health unit.

These forms should then be returned either to the local health unit or to the central location of the Arkansas Department of Health (4815 West Markham, Slot 44, Little Rock, Arkansas 72205) for registration. 

Electronic Submission

The Arkansas Department of Health encourages all birth and death records to be submitted electronically through ERAVE (Electronic Registration of Arkansas Vital Events).

Similarly, circuit clerks are encouraged to submit their monthly records of divorces electronically to the Arkansas Department of Health.

Timeframe for Submission

Whether submitted in paper form or electronically, all vital records from each month should be submitted to the Arkansas Department of Health no later than the 10th day of the month following.
