Navigating the Dental Hygiene Collaborative Care Permit Process in Arkansas

The application form for a dental hygiene collaborative care permit is issued by the Arkansas Department of Health’s Office of Oral Health, and is the first part of obtaining a permit. The collaborative team (dentist and dental hygienist) need to indicate the type of public setting and area of the state where work is intended to be practiced. On approval, the applicant will be directed to the Arkansas State Board of Dental Examiners for a written permit of agreement.
