Certain minor errors on certificates (e.g. some errors of spelling, of times and dates, etc.) may potentially be corrected without the need for a court order. However, due to the number of variables and possible errors, each case must be reviewed by our staff individually to determine what documents or actions will be required to make the requested amendment. Please note: a court order is required in order to correct an item that has previously been corrected, as a correction can only be made to an item once.
Who should I contact?
If the child listed on the certificate is under one year of age, please contact the hospital where the child was born and speak to Medical Records. They will then send an affidavit for correction to the Arkansas Department of Health in order to correct the birth certificate.
If the child listed on the certificate is over one year of age, please contact the Arkansas Department of Health at 4815 West Markham, Slot 44, Little Rock, AR 72205, either in person, by mail, by phone at 501-682-1214 or by email at [email protected]. Office hours are Monday through Friday, 8:00am to 4:30pm, with the exception of state holidays, when the office is closed.
Correct Minor Errors on a Death Record

If the error is in the top portion of the certificate (lines 1-17e), the funeral home can submit an affidavit along with payment to the Arkansas Department of Health at 4815 West Markham, Slot 44, Little Rock, AR 72205. However, if the error is in the medical portion of the certificate, the certifier will have to submit the affidavit and payment.
